A PDF does not store text as a simple document. It stores every word as a fixed position on a page, and paragraphs, columns, tables, and images are all separate layers. When a basic translation tool reads a PDF, it pulls out the raw text, discards all the position data, translates the words, and dumps them back out. The result reads correctly but looks nothing like the original.
Scanned PDFs are harder still. A scanned page is an image of text, not real text. Before any translation can happen, the tool needs to read the image with OCR — optical character recognition — and rebuild the text layer while keeping track of where columns, tables, and headings are.
